Community Health Nurse

The Community Health Nurse serves as a health counselor, health educator, health advocate, and health care referral agent to clients with Health Care needs. The Community Health Nurse serves clients in Huntersville, Cornelius, Davidson, and Mooresville. There are no citizenship or age requirements.

Special emphasis given to seniors who have questions regarding navigating through the Healthcare system, transitioning living arrangements, or the need for monitoring conditions such as Blood Pressure or Diabetes.

There are no fees for service. Please bring a form of ID.
